Referrals

To submit a referral for services currently under contracts with a county human/social service agency, please see link below.  

This page and referral process are for CCS and CLTS services that are not day programming (Day Camp and Community-Based Day Services).  Please see the Camp Clover Day Program tab for more information on that program and to make a referral.

PLEASE NOTE-ONLY COUNTY CASE WORKERS CAN SUBMIT A REFERRAL FROM THE PROGRAM THAT IS PAYING FOR SERVICES.
