FAMILY AND UNPAID CAREGIVER SUPPORT AND SERVICES
This service encompasses many things for providers. What Clover Family Services offers is:
IEP Planning and Advocacy before/during/after IEP meetings.
Supporting and helping parents/guardians understand the behaviors of their children and effectively responding to them with individual plans according to the needs.
3. Implementing routines, behavior management, and basic organization for parents/guardians.
The rate for this service for 2024 is $35.75/15 minutes.
SAFETY PLANNING AND PREVENTION
This service is provided under Children's Long-Term Support. This service offers planning for de-escalation techniques, and who to call in what steps when escalation happens at home, in school, or in the community. This service is 14 sessions over 22 weeks, provided there are no cancellations or delays. This service is executed with a Master or Bachelor Level provider to develop and monitor the Safety Plan, and an Associate or Rehabilitation Tech level provider to implement and assist with carrying out the safety plan.
The rate for this service for 2024 is $35.75/15 minutes for Master Level. The rate for the Associate/Bachelor level is pre-determined by DHS.
